The life of Samuel "Sammy" Mark Koscak, 22 was tragically ended on April 29, 2022.  He was born October 21, 1999, in Glendale, Arizona. He attended Central High School. He had a passion for physical fitness. He played youth sports and was a member of the Rushmore Hockey Association. He was an exceptional hockey player and his dad loved to boast about his abilities on the ice to anyone willing to listen. He enjoyed being an active member of Bethel youth and exploring his faith through reading.

He is remembered by his loved ones as a sweet little boy with a head full of curls, chubby cheeks and a smile that would melt your heart.  He is known by his family to have a quick wit and the ability to make any situation fun. He was loved by his family and many friends, but he was especially loved by his sisters and cousin Jack.

He is survived by his dad and stepmom, Matt & Theresa Koscak; his mother, Fawna Augustine; sisters, KT, Victoria (Vicki), and Matejah (Matty) Koscak; his stepsiblings, Morgan, Megan, and Mason Russell; his paternal grandparents, Bob and Judi Koscak; his maternal grandparents, Mark and Tina Augustine; great grandparents, Paul and Sonya Taylor; his aunt, Pamela Heinen; uncle, Eric Koscak; uncle and aunt, Chad and Marta Koscak; aunt, Destiny Larson; uncle & aunt, Cody & Christine Kruger, and too many cousins to count.

A Celebration of Life service will be held May 6 ^th , 2022, at 6 PM at Bethel Church, 1202 N.
