Cremation Authorization Form

A Cremation Authorization can be signed in either an At-Need or a Pre-Need situation. If the form is being signed for a person who is prearranging, they may either sign for themselves, or if their health prevents them from doing so, a person designated as the Power Of Attorney may sign on their behalf. If the form is being signed in an At-Need situation, the surviving next of kin must sign the form. In all cases, the form must be signed in front of a licensed funeral director, or must be signed in the presence of a notary public and must be notarized.

 

A Cremation Authorization is the necessary paperwork required to give the funeral home permission to perform a cremation. If pre-planning for a cremation, it is best to have a signed Authorization on file.
